Eligibility - Population(s) Served: Registered students up to age 25 as defined under the Education Act who have a valid Ontario Health Card, has assessed needs for nursing, occupational therapy, physiotherapy, speech language pathology or diabetic services and requires those services to attend school, receive instruction or participate in school routines and who would experience a significant disruption in attendance, instruction, or participation without support of the program
