Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
 
Home At Last - Identifies older adults in local hospitals who have limited supports upon discharge
 
Home Help Program - Assistance with daily chores, such as laundry, light housekeeping, errands, grocery shopping, and meal preparation * home helpers visit weekly, bi-weekly, monthly (every 4 weeks)
 
SMART Falls Prevention and Group Exercise Classes for Seniors - Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls
 
Volunteer Visiting - Support program that matches volunteers with a senior or adult with a disability for one-on-one visits in the client's home or a weekly chat over the phone * visits are regular and may also include enjoyable activities outside of the home
 
Wellness Link - Programs to support independent living for seniors and adults with disabilities are offered at London Housing Corporation rent-geared-to-income apartments
