Service Description: Mental health case management to people not able to receive support by existing community resources

Services include time limited, goal-specific community treatment, assistance with medication issues, relapse prevention, psychoeducation, liaison with community organizations and advocacy
Fees: Most fees are covered by OHIP
Application: Referrals can be made by doctors, social workers, case workers, nurses, community agencies, and families
Eligibility - Population(s) Served: Youth 16 years and older and adults up to 65 years with serious mental health issues
