Service Description: Outpatient addiction assessment treatment, referral and follow-up services

Day Program - 3-week program, Mon-Fri in a group format * emphasis on psycho-educational and cognitive behavioural approach that includes developing and reinforcing skill development within a context of relapse prevention * limited number of supportive beds for those clients who require housing while in treatment

Structured Relapse Prevention Group - Offered one evening per week as a follow-up to the Day Program * clients who have attended other addiction programs may also apply for this group
Fees: Services are covered by OHIP
Application: Family or medical referral * assessment interviews required
Eligibility - Population(s) Served: Adults and youth 16 years and older who have substance use problems and want to make a change
