Service Description: Support for Adults 18+ living with diabetes * one-on-one or group consultation on monitoring blood sugar levels * medications * healthy eating * exercise and other lifestyle choices * Certified Diabetes Educators, Registered Nurse and Registered Dietitian on staff * Certified Assistive Devices Program (ADP) Centre:
  • individual and group counseling
  • medication adjustment
  • insulin initiation and adjustment
  • insulin pump therapy
  • gestational management
  • glucometer teaching
Insulin Pump Program:
  • education and support for adults 18+ with type 1 diabetes who are using or considering using an insulin pump

Eligibility - Population(s) Served: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes, and gestational diabetes
