Service Description: One-on-one and group supports for people living with mild cognitive impairment, Alzheimer's disease and other dementias, and their families and care partners

First Link Learning Series - Regulated health professionals provide a comprehensive overview of dementia, coping strategies, resources and support systems * participants are offered an opportunity to share and connect with others embarking on the same journey

Social Work Support - Registered Social Workers provide in-office, in-home, telephone or virtual assessments and work with all those involved to develop an individualized response to needs * may include individual and family counselling, advocacy and referral to community resources

Support Groups - Monthly support groups for care partners and those in the early stages of dementia * facilitated by trained health professionals groups allow clients opportunities to share, support and learn from one another * in-person and virtual groups available
