Service Description: Information, support and assistance to persons living with Alzheimer's disease or other dementias and their care partners

Advance Care Planning - Assists people living with dementia and their partners in making decisions for future care including financial and personal health care plans

First Link Learning Series - Offers persons and families living with dementia a progressive educational series that focuses on learning about dementia and strategies to manage care 

Learning the Ropes for Living with Mild Cognitive Impairment - 6-week program that focuses on memory strategies for people living with a diagnosis of Mild Cognitive Impairment and their care partners

Taking Control of Our Lives- 9-week topic specific group for people living with a diagnosis of Alzheimer's disease or other dementia

U-first for Care Partners - This is a 6 hour program designed to educate the care partner to build skills and confidence caring for their loved one

Support Groups - Groups for family caregivers held both in person and virtually in various locations including Clinton, Exeter, Goderich, Grand Bend, Listowel, Seaforth, St Marys, Stratford, Wingham, and Online * please call for specific details

Social Groups - Support and activity group for people in the early-middle stages of Alzheimer's disease or other dementia that are offered concurrently with a Caregiver group
