Service Description:

Community-based support and rehabilitation services for adults living with the effects of an acquired brain injury
|
Assisted Living Services
- Residential care for adults with an acquired brain injury * focuses on increasing and maintaining skills and improving quality of life * service length runs from short-term or life-long
|
|
Community Transitional Services
- One-on-one transitional support, rehabilitation, and service coordinator for adults living with an acquired brain injury * focuses on skills training, improving quality of life, and connecting clients with other service providers * service offered in the community or the client's home * services are offered for up to 12 months
|

Consultation and Training
- General education for care partners and service providers about acquired brain injury * training for individuals and teams to support and interact more effectively with clients who experience behavioural, social, and cognitive challenges related to acquired brain injury
|

Group Services
- Available across the South West Region, Group Services are flexible and provided in a group setting focusing on individual and group goal achievement, increased independence and quality of life * groups provided offer social, recreational, wellness, skill building, exercise and therapeutic activities * services are available on a short-term, transitional or lifelong basis
