Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * culturally appropriate services for Indigenous adults

Transportation - May assist with transportation to medical appointments and centre programs

Friendly Visiting - Regular visits to clients in person or by telephone

Caregiver Support
- One-on-one and group support

Volunteer Hospice Visiting Service - Support service for clients that are in palliative care and their caregivers

Indigenous Support - Supports provided through information, referrals, advocacy, and assistance to access health care services within the local urban area
