Service Description: Supports for people with chronic and acute illness, disabilities * helps people remain independent in their own homes for as long as possible * culturally appropriate services for Aboriginals

Long Term Care Program - Congregate dining, outings and adult day programming for elders and disabled individuals

Aging at Home Program - Assists elders over 55 years of age with arranging Power of Attorney, wills, utility plans, minor home repairs, assistance with transportation to dental and hearing appointments and social visiting

Home and Community Care Program - Care Management by RNs and RPNs

In-Home Personal Support and Homemaking Services
- Supports for the activities of daily living, including personal care, meal preparation, laundry, light housekeeping, medication compliance, blood sugar and blood pressure monitoring, simple dry dressings, escorted shopping
