Service Description: Multi-agency team that provides access to core mental health services: information and referral, mental health counselling, intensive case management, housing and outreach support, social recreational activities and leisure access support services

Outreach Community Support - Support program for adults with serious mental illness living in the community:
  • daily living skill development
  • supportive counselling
  • linking with other formal and informal supports
  • advocacy social/recreational enrichment
Fees: Free * fees for some social activities

Eligibility - Population(s) Served: People 16 years and older with a serious mental illness and their families
