Service Description: 22-bed inpatient program providing time-limited, intensive rehabilitative care for people with complex medical and physical conditions * treatment focuses on patient's ability to improve function and independence in activities of daily living in order to return to a community-based setting (home, retirement home)

Application: Referral required from a primary healthcare provider (physician or nurse practitioner)
Eligibility - Population(s) Served: People in need of rehabilitative care related to complex medical and physical conditions * must be able to actively participate in and tolerate 2-3 hours of therapy per day * can be admitted from community, another hospital or following an inpatient admission, once any acute medical issues are stabilized
