Service Description: Care and support for children and families living with life-threatening or life-limiting illness or disease * multidisciplinary team of physicians, nurses, social workers, child life therapists and spiritual advisors

Services include:
  • pain and symptom assessment and management
  • written Advance Directives
  • connections to community palliative care agencies and services
  • psychosocial support and written resources for patients, parents and siblings
  • bereavement care
  • assistance with creation of memories and legacy
Fees: Most services covered by OHIP
Application: Call for more information * medical referral required for most services
