Service Description: Provides assessments, counselling, discharge planning services, information and education

Patients can counsel with social workers about:
  • adjustment to illness and treatment
  • relationship and family issues
  • stress and conflict
  • grief and loss
  • end of life care issues
  • parenting issues
  • discharge planning, making connections to hospital and community-based resources, and home supports
  • communication needs and issues with the health care team
  • issues related to physical, emotional and sexual abuse, and neglect

Application: Walk in for emergency department * medical referral required for most other services
