Service Description: The eHomecare model of Nursing care utilizes specially trained personal support workers (known as Health Care Technician (HCT)) who receive support from a remote Directing Registered Nurse (DRN) through a web-based portal. The HCT carries out delegated activities, on behalf of the DRN, while providing personal care to the patient. This allows the patient to stay in their home environment or until transitions to alternate level of care facility (ex: Long Term Care Home, Hospice).

Hospice at Home utilizes the eHomecare model of care, but is specifically for patients with an end-of-life trajectory.  There are a set number of funded beds established in each region throughout the South West.

Eligibility - Population(s) Served: Eligibility - Care Coordinator will outline service to client/caregivers and establish eligibility and consent. Target Population -eShift – chronic palliative and complex; all ages -Hospice at Home – adults 18 and over, palliative w/ an end-of-life trajectory of 2-3 months
