Service Description: 10-bed hospital inpatient rehabilitation program for older adults * focuses on activation, physiotherapy, and socialization for patients who are awaiting discharge to an alternative destination, such as a long-term care home * services include personal support, occupational therapy, and communal dining and activities
Fees: Covered by OHIP
Application: Patients at all South Bruce Grey Health Centre hospitals are screened for program eligibility once they have completed the acute portion of their stay and are awaiting discharge to an alternate destination
Eligibility - Population(s) Served: Geriatric patients who are no longer being treated for an acute illness and are awaiting discharge to an alternate destination, such as long-term care, or home with community supports
