Service Description: Teams provide comprehensive integrated services to adults living with a severe and persistent mental illness

Services include:
  • assessment
  • individualized treatment plans
  • coordination of care and services for clients
  • one-on-one counselling and support
  • life skills training
  • monitoring of the mental state of clients and their use of medications
  • vocational and recreational activities and programs
  • family education and support
  • referral and advocacy services to community agencies
