Service Description: Peer Outreach Workers use their own lived experience with mental health and addiction to walk alongside people in their journey of recovery and wellness * supportive listening and assist with navigating the mental health, addiction, and health care systems

Support people admitted to hospital for mental health or addiction-related concerns, those attending addiction medicine clinics, as well as meeting with people in the community * support connecting to services including attending appointments, accessing employment and education supports, as well as any number of other tasks that support identified goals as part of the recovery journey

Eligibility - Population(s) Served: Adults, youth and families who are affected by substance use and dependence, gambling/behavioural issues, and/or mental health concerns
