Service Description:

Primary health care clinic for registered patients only (not a walk-in clinic) providing chronic disease management - diabetes, hypertension, hypercholesterol, obesity * mental health counseling, assessment and referral * addictions assessment, referral, counseling, after-care counseling
Team includes:
- nurse practitioners
- consulting physician
- registered practical nurses
- chronic disease coordinator
- social workers
Community Programs (open to the public)
- Oxford Addiction Information, Walk-In and Treatment Services (OATS) - Addiction assessment and referral
- Canadian Mental Health Association (CMHA) Walk–in Counselling - Tue 3pm-7:30 pm
