Service Description: 12-bed acute care hospital with emergency department * medical, day surgery, therapy and ambulatory care services * case managers from Ontario Health atHome South West area available on site

Part of the Middlesex Hospital Alliance (in partnership with Strathroy Middlesex General Hospital)


Ambulatory Care Clinics - Outpatient specialist clinics providing diagnoses and treatments for various medical conditions

Community Support Services - Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

Diabetes Education Program - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Diagnostic Imaging Services - Outpatient and inpatient services involving various tests to diagnose medical conditions

Emergency Rooms - Emergency room care * doctors, nurses, and other health professionals assess and treat life-threatening injuries and severe illness

Medical Surgical Day Services - Two surgical suites are used to provide general day surgeries on an outpatient basis and not involving an overnight stay

Memory Clinic - Clinic specializing in assessment and care for people with memory disorders * holistic inter-professional care and multidisciplinary team, including physician, nurse practitioner, social worker, pharmacist, occupational therapist, registered nurse * initial assessments may take up to 2 hours * family and caregivers included in assessments

Palliative Program - One inpatient room for palliative care * pain and symptom management * guidance, support and connections to community resources for patients and families * continuing education on pain, symptom management and end-of-life care for care team

Physiotherapy Department - Community physiotherapy clinic in a hospital * health professionals assess and treat conditions that affect the muscles, nerves, and bones that may be caused by work or sports or motor vehicle collision injuries, chronic conditions, or to recover after an operation (hip and knee surgery)

Fees: Most fees are covered by OHIP
Application: Walk-in for emergency department * medical referral and appointment required for most other services
