South West Regional Wound Care Framework Iniative
Online Toolkit Supports Excellence in Wound Care Delivery Across the South West

When the results of a South West Community Care Access Centre study identified opportunities to improve client outcomes and resource utilization in the delivery of wound care, representatives from acute care, long term care and community care came together.

The result of this partnership? A resource to support the provision of excellent and evidence-based wound care delivery across the South West regardless of geographic location or health care sector: woundcare.thehealthline.ca

The toolkit provides a collection of resources and educational components, including: background and “how-to” information, assessment tools, algorithms, clinical information, teaching handouts and literature resources. All of the materials in the toolkit support evidence-based, leading practice in wound care, and it is hoped that this will become a “go-to” resource for all health care providers.
