New Regional Community Partnership Coordinators Join thehealthline.ca

The South West Community Care Access Centre and thehealthline.ca, an innovative health services portal, have hired three new staff members to play a key role in providing South West CCAC clients with information about important health and support services. As Regional Community Partnership Coordinators, the three are dedicated to building stronger partnerships across the region in order to enhance information provided through thehealthline.ca.

“They will reach out to community organizations to collect and share information, helping to maintain thehealthline.ca’s reputation for credible, up-to-the-minute data,” says South West CCAC Executive Director Sandra Coleman. Ultimately, the Coordinators will identify new opportunities for community partnership to support the sharing of information across the region.

Lise Baronet Kosmack will serve in London-Middlesex and Elgin Counties. Lise has worked in information and employment services, with the London Public Library and as Manager, Placement Services, Medix School in London. She also has networked extensively with health care providers throughout the South West. “I’m excited about this opportunity to help,” says Lise. “I look forward to partnering with our diverse community of health care providers in order to empower people with information that will make a positive impact on their care experiences.” Lise can be contacted at 226-234-7085 or email lise.kosmack@sw-ccac.ont.ca.

Celina Thomas Hicks will focus her efforts in Huron, Perth and Oxford Counties as well as west Norfolk. Celina has many years experience in social services, most recently as Executive Director of Victim Services of Perth County. She also serves on the Police Services Board of St. Marys, Ontario. “It’s great to be involved with the South West CCAC and an innovative service like thehealthline.ca,” Celina says. “I’m looking forward to working with our community partners and learning more about the needs of individual communities.” Celina can be contacted at 519-801-5654 or email celina.thomashicks@sw.ccac-ont.ca

Teresa Zohorsky will be the Grey-Bruce Coordinator. She has an extensive background in marketing and communications, most recently working as Social Marketer for Health Promotion, Grey Bruce Health Unit. Teresa says, “I’m looking forward to meeting and working with local agencies and providers to ensure we deliver the best information about the wealth of services in our area.” Teresa can be contacted at 519-374-8008 or email teresa.zohorsky@sw.ccac-ont.ca effective February 21, 2008.

“We’re very fortunate to have attracted such outstanding people to our team,” says Michael Robbins, Project Coordinator of thehealthline.ca. “And we’re particularly pleased that their experience is so diverse – Lise, Celina and Teresa really cover all the bases. We expect great things!”

The three Coordinators began work in February.

The South West CCAC serves nearly one million people living in Grey, Bruce, Perth, Huron, Middlesex, the City of London, Elgin, Oxford and part of Norfolk County. CCAC case managers help people to live independently at home and to apply for admission to a long-term care home. The CCAC also provides information about local health-related agencies and links people with these services.

thehealthline.ca is a health services portal listing health-related organizations and services across the South West and providing a variety of other information services. In 2007, the site received more than 1.5 million visits.
