Service Description: The Apartment program is affordable transitional housing up to 4 years that provides opportunities for individuals with a longstanding serious mental illness and/or substance use involvement to stabilize and establish connections with the community.

Program offers:
  • Assessment and planning 
  • Support with achieving recovery directed goals
  • Direct service provision/intervention
  • Monitoring, evaluation, and follow-up
  • Information, liaison, advocacy, consultation, and collaboration
  • Planning for crises and helping individuals to develop coping mechanisms
Goals & Objectives:
  • Support clients with serious mental illness and/or substance use involvement who are homeless, at risk of becoming homeless or are inadequately housed with a safe living environment to work towards recovery goals
  • Support clients in their search to find and keep appropriate housing while developing skills for independent living
Fees: Free * rent geared-to-income for residences
Application: Application and referral required for geared-to-income housing * self or other service provider referral for outreach support
Eligibility - Population(s) Served: Adults 18 years of age and older who:
  • want to make a commitment towards mental health and/or substance use recovery
  • are willing to regularly engage with the worker (This is a requirement)
