Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurses, dietitians, registered social worker, endocrinologist, pediatrician and a nurse practitioner on staff
Outreach clinics at Bruce Power, Chippewas of Nawash Unceded First Nation, Saugeen First Nation

Diabetic Foot Ulcer Clinic
- Assessment and treatment of diabetic foot ulcers * assessments by a Nurse Practitioner with access to a multi-disciplinary wound care team * specialized devices to take pressure away from the foot ulcer offered at no charge

Insulin Pump Program
- Team of registered nurses and registered dietitians provide education and support for adults and children with type 1 diabetes who are using or considering using insulin pumps and continuous glucose monitors

|
Pediatric Program
- Team of registered nurse, registered dietitian and social worker provides education and coaching in self management to children with diabetes and their families
