Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring:- blood sugar levels
- medications
- healthy eating
- exercise
- other lifestyle choices
Staff include:- registered nurse
- registered dietitian
- nurse practitioner (all certified diabetes educators)
