- Community-based support and specialized programs for adults with physical or developmental disabilities, including those who are medically complex and Chronic Mechanical Ventilation Dependent

- Centre-based day supports for adults with physical disabilities and complex medical needs * individually-designed support and assistance provided in all areas, including activities of daily living, leisure and social activities, as well as community participation

- Support and assistance can be time-limited to address a specific need or request or can be flexible and ongoing * direct support as well as planning and facilitation to assist families to manage funding * assists in matching support workers with families * support to workers and families during initial stages and ongoing support to find meaningful activities in the community * system navigation support
