Service Description: Offers a variety of services that assist eligible persons to remain healthy and independent in their own homes

Assists with the costs of certain services such as:
  • grounds maintenance
  • housekeeping
  • nutrition services such as Meals on Wheels
  • health and support services
  • transportation services
  • care services such as nursing or personal assistance
  • home adaptations
  • ambulatory health care to services outside the home

Application: Call, meet with counsellor or visit website for forms and detailed information * services provided in veterans homes and at office locations
Eligibility - Population(s) Served: Veterans who need home care services and have qualified for a disability benefit, for the War Veterans Allowance, or receive Prisoner of War Compensation, orare eligible for, but can’t access, a bed in a long term care facility
