Service Description: Community health care agency * Personal Support Manager available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home
  • bathing/assistance with personal care
  • dressing assistance
  • mobility assistance
  • meal preparation assistance

Nursing services are available at various clinic locations

Please call our office for clinic information. Clinic nursing services provided include:
  • wound care
  • IV therapy
  • catheter care
  • drain/suture/staple care
  • health teaching regarding diagnosis/disease self-management/medication management
Nursing care in-home where patients meet the exclusion criteria to attend clinic

TB testing available at our Owen Sound and Walkerton clinic location.
