Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups available on and off reserve

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health * culturally appropriate programs and services for First Nations, combining traditional healing and Western medical practices
 
Chronic Disease Management - Promotes healthy living, diabetes prevention and self management through nutrition education and physical activity to community members of all ages
 
Community Health - Public health programs and services that involves health promotion, illness and injury prevention, and health maintenance
 
Home and Community Care Program - Supports for people with chronic and acute illness, disabilities * helps people remain independent in their own homes for as long as possible * culturally appropriate services for Aboriginals
 
Human Wellness Services - Promotes recovery and healing in safe environment * incorporates holistic and traditional cultural practices with contemporary western approaches * collaboration with community-based programs and external services * NNADAP counsellors
