Service Description: Finding Your Way Ontario provides public education and an ID kit to assist in a search for missing persons. With consent, ID kits are completed by registered clients and faxed to local police for entry into their database, so all relevant information about the person living with dementia is already stored and ready to be used in the event of an emergency. Contact your Social Worker or contact@alzswp for more information.
