Service Description: 62-bed inpatient rehabilitation program for adults who have ongoing complex care needs that cannot be meet in the community or in a long-term care home

Short-Term Complex Medical - For patients who are currently not eligible for admission to long-term care * anticipated length of stay is up to three months

Long-Term Complex Medical - Provides medically complex and specialized services over an extended period of time to maintain, reduce the progression of, or avoid further loss of function
Fees: Services covered by OHIP * co-payment fee required
Application: Medical referral required
Eligibility - Population(s) Served: Short-Term Complex Medical: Medically stable patients with a complex illness or disability that requires skilled, technology-base care not available at home or in long-term care, and are not currently eligible for admission to long-term care
Long-Term Complex Medical: Medically stable patients with a long-term illness or disability that requires ongoing assessment and active care management by a specialized interprofessional team * patient's care needs cannot be met on an ongoing basis in long-term care or other community setting, resulting in the patient needing residential care in hospital
