Service Description: Time-limited hospital-based outpatient rehabilitation program for people recovering from stroke/neurological conditions * services may include:

  • physiotherapy
  • occupational therapy
  • social work
  • speech-language pathology
Fees: Covered by OHIP
Application: Medical referral required
Eligibility - Population(s) Served: People in need of rehabilitative care for stroke/neurological conditions, excluding spinal cord and acquired brain injury * must have specialized neurological treatment needs that cannot be met by community services * must have potential for functional and/or neurological improvement and would benefit from a goal-oriented, time-limited rehab program * rehab goals must be specific, measurable, attainable, realistic, and timely * must be able to learn and incorporate self-management strategies to continue therapy at home * must be medically stable and have sufficient tolerance to participate meaningfully in therapy sessions (travel to outpatient clinic included) * must have caregiver support for personal care while attending therapy if required * must have reliable transportation
