Service Description:

Care and support for children and families living with life-threatening or life-limiting illness or disease * multidisciplinary team of physicians, nurses, social workers, child life therapists and spiritual advisors
Services include:
- pain and symptom assessment and management
- written Advance Directives
- connections to community palliative care agencies and services
- psychosocial support and written resources for patients, parents and siblings
- bereavement care
- assistance with creation of memories and legacy
