Service Description: Affordable transitional housing for up to one year as part of an addiction treatment program for people with a long-standing drug or alcohol problem * opportunities to stabilize and establish connections with the community * holistic, strengths-based approach * supported by Outreach Support Workers * helps clients find permanent housing and maintain it for the long term while working on recovery goals

Program includes:
  • assessment and plan
  • support with achieving recovery-directed goals
  • direct service provision/interven
  • monitoring, evaluation, and follo
  • information, liaison, advocacy, consultation, and collabora
  • planning for crises and helping develop coping mechanisms
Fees: Cost for apartment rental * clients must have a source of income through employment or social services support (Ontario Works or Ontario Disability Support Program) and be eligible for rent support

Waitlist depending on current occupancy status of each unit and the number of other qualified applicants on the list
Eligibility - Population(s) Served: Adults aged 18 years and older who want to reduce or eliminate harms associated with substance use and have a willingness and a commitment to work on personal recovery goals
