Service Description:

Support for young adults who are, or have been, involved in the Child Welfare system and are transitioning to independent living * connects individuals with resources available in their community
Assists young adults in identifying, developing and implementing a plan based on personal goals including strategies to address:
- Educational resources and post-secondary opportunities
- Employment services and training
- Life skills training
- Health, mental health and other social services supports
- Volunteer and recreational activities
- Housing
- Safety planning
- Legal services
