Service Description: Primary care services to people who either lack primary care or require more comprehensive primary care than they are currently able to receive from their family physician or nurse practitioner * available to people with a serious mental illness or addiction issue for which they are receiving local mental health or addiction services

Nurse Practitioner and Registered Nurse offer variety of services, including, but not limited to:
  • comprehensive health assessment
  • monitoring and management of chronic diseases such as diabetes, arthritis, asthma and heart disease
  • prescribing and monitoring of medication
  • immunizations
  • health teaching
  • referral to specialists and other human services
  • consultation and service coordination

Program is delivered in collaboration with the Brockton and Area Family Health Team and four physician groups * program is based in Walkerton and Owen Sound with outreach to Durham, Hanover, Chesley, Paisley, Kincardine, Southampton, Wiarton and Port Elgin

Application: Referrals accepted from healthcare providers such as mental health/addictions workers
Eligibility - Population(s) Served: Adults with a serious mental illness or addiction issue for which they are receiving local mental health or addiction services
