Service Description: Mental health, addiction and problem gambling assessments, treatment and follow-up. Offers individual, family and group treatment.
Application: Referrals must be made by physicians or nurse practitioners. Please fax the completed Centre for Mental Health referral form. Eligibility criteria: - People accessing this service must be Portuguese language-dependent - Patients must reside in our catchment area: Parkside Drive to the west, Yonge Street to the east, Lake Ontario to the south and Eglinton Avenue to the north - Referrals are accepted for individuals who cannot access English language services due to a language barrier
