Service Description:

Community-based health care in which paramedics provide alternative to family doctor, walk-in clinic, or emergency department * works in collaboration with patient’s primary care provider to conduct preventative, at-home health care visits * focuses on reducing avoidable use of emergency and ambulance services * helps older adults and vulnerable, at-risk people live safely in their homes * services include:
- chronic condition management
- palliative pain and symptom management
- wound care management
- point-of-care testing
- prescription medication/vaccine administration
- remote patient monitoring
- care coordination with health care providers
- referral to community programs and resources

High-risk clients who have complex medical issues such as diabetes, chronic obstructive pulmonary disease, and congestive heart failure Adults who are on the waitlist for long-term care, have been assessed as eligible for long-term care, are soon eligible for long-term care, or on the long-term care crisis waitlist
