Service Description: Assessment, treatment and referrals of people with substance use, internet, gaming and problem gambling issues * services include crisis intervention, virtual supports, mobile crisis teams, police partnerships, case management, counselling, community wellness programs, housing supports and addiction medicine supports * treatment and service plans are developed based on standardized screening and assessment
 
Addiction Transitional Case Management Service - Assessment and Referral - Program facilitates discharge process from London Health Sciences Centre and St Thomas Elgin General Hospital * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care
 
Back On Track Program - Provincially mandated program for convicted impaired drivers and drivers who receive two or more provincial alcohol or drug related driving offences
 
Clinical Services - Health promotion, disease and injury prevention and chronic disease management to clients
 
Community Opioid Addiction Program - Offers direct client services and community education for people with opioid addictions * assessment and screening required for entry into the treatment program
 
Community Wellness - Variety of leisure and life skills focused groups and activities offered to support personal growth and recovery
 
Community Withdrawal Support Program - Provides comprehensive community withdrawal management, addiction and mental health crisis care
 
Family Support Programs - Program provides family members, caregivers and friends with tools to help in the recovery process of a loved one
 
French Mental Health and Addiction System Navigation Program - Provides access to mental health and/or addiction support services in French
 
Heartspace Program - Specialized prevention, early identification, education and treatment program for women who are pregnant, parenting and substance-involved and their children (prenatal to six years of age)
 
Justice Community Support Program - Mental health community support with an emphasis on short-term support and specialized support for those with justice involvement
 
London Coffee House Program - Drop-in centre that provides free coffee, tea, juice and a light snack as well as one-on-one support
 
Mental Health Public Education - Mental health education, promotion and resources for the general public
 
Peer Support Program (OCD) - Mutual support group for people with Obsessive Compulsive Disorder (OCD)
 
Problem Gambling and Behavioural Addictions Program - Treatment planning and community-based outpatient treatment for people with gambling, gaming, internet/technology, shopping/overspending or pornography/sex concerns
 
Rapid Access Addiction Medicine (RAAM) Clinic - Community-based, low barrier services for clients seeking treatment for any substance use disorder * focuses on addiction medicine and offers both medication and psychosocial interventions * community education about addiction medicine for health care professionals
 
Smoking Treatment for Ontario Patients (STOP) Program - Delivers smoking cessation treatment and counselling support to registered clients who wish to quit smoking
 
Substance Use Program - Treatment planning and community-based outpatient treatment for people with a substance use concern
 
Transitional Case Management Services - Assessment, development and implementation of rehabilitation and recovery plans * focus on daily living and housing, health and wellness, relationships, finances, access to employment and education, and community involvement
