A South West CCAC/UWO Social Interaction Implementation Project

Accelerating Knowledge to Action (KTA) on Client-Driven Care was a collaborative 2-year project (April 2006-July 2008) to mobilize and test shared effort to use research findings on how to partner with clients and all involved in in-home care in South Western Ontario.

This project worked to refine care partnering relationships by using tested practice approaches that research has found to improve the health and independence of in-home clients and the contribution of knowledge, skills and involvement of everyone on the care team, including clients.

Anyone who needs a test can get one and does not need an OHIP card. Tests are made available by appointment only at assessment centres and at pharmacies
