Advance care planning is a process that guides a person to talk about their wishes, values, and beliefs that would inform future decisions when a person is unable to make or communicate their own healthcare choices.

Anyone who needs a test can get one and does not need an OHIP card. Tests are made available by appointment only at assessment centres and at pharmacies
