Home and Community Care Review Expert Group Survey

The Minister of Health and Long-Term Care has appointed a Home and Community Care Review Expert Group to develop recommendations addressing three areas across the home and community care sector. Home and Community Care involves any services delivered at a person’s home to support them in maintaining optimal independence and health in the community.  Areas to be addressed by the Expert Group include:

1. Opportunities to make service delivery more consistent across the Province in order to promote equal access to quality home and community care and services across all communities;

2. Opportunities to make provincial funding for services and investments in home and community care services more consistent across the Province in order to improve transparency, and maximize the funding available for patient/client care and services;

3. Opportunities for innovations and new approaches to care in order to improve the patient/client experience, quality, choice and value for money.

To inform these recommendations the Expert Group is seeking input - via a survey - from people living in the South West LHIN on the single most important thing that the Ministry of Health and Long Term Care could do to improve home and community care for them.
