Service Description: Range of services and supports for older adults and adults with disabilities * helping people remain independent in their own homes and community for as long as possible

Assisted Living in Supportive Housing and the Community - Supported living option includes assistance with daily living skills, such as personal care, meal preparation, medication monitoring, budgets, house management, and community involvement

Foot Care - Basic foot care provided by trained personnel in the client's home, retirement home or hospital and with additional foot care clinic locations  in The Livingston Centre, Ingersoll Services For Seniors, Delhi Legion, and Norwich Community Medical Centre. Foot care includes,  care of corns, calluses, trimming nails, applying lotion * for non-insulin and insulin using people with diabetes and those on blood thinners

Home at Last (HAL) - Assists older adults with transitioning from hospital to home

Housekeeping - Assistance with daily chores, such as laundry, light housekeeping, shopping, errands, meal preparation, and special needs specified by the client * trained personnel visit weekly, bi-weekly, monthly, or seasonally

Meals on Wheels - Hot meals delivered by volunteers to client homes * special diet types available * service includes client safety check

Transportation - Non-emergency transportation to medical appointments and regular activities such as shopping, or to attend social activities * volunteer drivers
