Service Description: Living alternatives to hospitalization in a home-like setting with staff support
  • individual and/or family units
  • shared living with low to moderate support
  • shared living with high support
  • youth (16-24) transitional shared living and individual units
  • women only – individual units
  • addictions supportive individual units
  • justice units

  • St Thomas
  • Woodstock
  • Exeter
  • Strathroy
  • Tillsonburg

Some individual units in other rural communities within Elgin, Middlesex, and Oxford counties, such as: Aylmer, Ingersoll, and West Lorne
Fees: Fee varies per housing type
Application: Complete a self-referral form * additional doctor or other service provider documentation may be required
Eligibility - Population(s) Served: Adults and youth 16 years and older with persistent mental illness who are characterized as complex with a history of lengthy and/or multiple hospital stays, have rehabilitation potential and readiness, as well as a desire for improved mental health
