Provides on-site personal support services to eligible individuals
Helps with daily activities and supportive services to maintain independence in the community at designated buildings * a hub model providing support to the community as well as the designated buildings
Services include: - assistance with personal care
- meal preparation
- medication reminders
- housekeeping and laundry.
Frequent visits, both scheduled and unscheduled, are provided throughout the day.
