Must call the care coordination service to provide necessary information * a doctor or nurse practitioner will determine if you meet the eligibility requirements

Eligibility - Population(s) Served:
|
- Must be 18 years of age or older
- eligible for publicly funded health care services in Canada
- be able to provide informed consent
- have a grievous and irremediable medical condition
- capable of making health care decisions
- voluntarily request MAID
