Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
 
Adult Day Services - Adult day program that offer supervised programming in a group setting and assist frail elderly and those with chronic illness or cognitive disability. Program also include assistance with personal care – such as bathing and foot care.
 
Assisted Living - Provides 24-hour on-site personal support services to eligible individuals
 
Dining for Seniors - Offers a hot meal and conversation
 
Exercise and Wellness - Focuses on seniors maintaining and/or improving their overall health
 
Foot Care Clinics - Basic and advanced foot care provided by nurses certified in advanced foot care management
 
Friendly Visiting and Telephone Reassurance - Friendly Visiting - Regular volunteer visits to older adults and people with disabilities who live alone or are identified as someone who would benefit from this program * volunteers are matched one to one with older people
 
Home At Last Program - Assists older adults with transitioning from hospital to home
 
Home Help - Services are customized to meet various needs and can include:
  • Light House Cleaning
  • Heavy Cleaning/Home Maintenance
  • Laundry
  • Grocery Shopping and Errands
  • Meal Preparation
  • Medication Reminders
  • Escort to Appointments
  • Grocery Delivery
trained staff visits work with the client's schedule and preferences * daily, weekly, bi-weekly and monthly
 
Meals on Wheels - Hot meals delivered Monday-Friday at noon * nutritionally balanced meal delivered to your door by screened volunteers * special diet types available
 
ONE CARE Lifeline Emergency Response - Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
 
Personal Support and Homemaking Services - In-home assistance provided by Personal Support Workers (PSW)
 
Respite - Overnight relief for caregivers of older adults, frail elderly and those with Alzheimer's disease and related dementias * accommodations are offered for either one or two nights at the Wingham and Goderich Adult Day Centres * trained staff provide assistance with activities of daily living, personal care and structured activities in a safe environment
 
Transportation - EasyRide - Non-emergency transportation to local medical appointments and regular activities such as shopping, personal errands and social activities * volunteer drivers * wheelchair accessible vans, cars, and taxis
