Service Description: Support program that matches a volunteer with people living with a life-threatening illness and bereaved families and friends in their homes, hospitals or long-term care facilities * non-medical service offers comfort, care, and emotional support to enhance quality of life

Regularly scheduled visits offer companionship, encourage activities of interest or assist with writing letters and creating legacy projects * volunteer give support and respite (breaks) to caregivers

Application: No referral required
Eligibility - Population(s) Served: People who would benefit from end-of-life compassionate peer support, and care partners who require bereavement support

Area(s) Served: East/West Perth County including Milverton, Millbank, Poole, Mitchell, Dublin, and surrounding rural areas
