Service Description: Community Support Programs help older adults and adults with special needs to remain independent in their own homes for as long as possible
 
Meals on Wheels - Meals delivered by volunteers to people unable to cook or shop for themselves * delivery also includes social contact and security check
 
Palliative Care Visiting Program - Support program that matches a volunteer with people living with a life-threatening illness and bereaved families and friends in their homes, hospitals or long-term care facilities * non-medical service offers comfort, care, and emotional support to enhance quality of life
 
SMART Group Exercise Program - Weekly 1-hour exercise class specifically designed for seniors and adults with disabilities
 
SMART In-Home Exercises Program - Provides a volunteer who goes in to the client’s home for one-on-one weekly exercise sessions
 
Special Steps Alzheimer Walking Program - Volunteers are trained, supported and matched one-on-one with person with dementia, such as Alzheimer's Disease, living in the community * volunteers offer support and friendship to the client through regularly-scheduled walks at indoor walking facilities or outdoors (weather permitting) or through regularly-scheduled sit-down visits
 
Volunteer Friendly Visiting Program - Regular volunteer visits to older adults and people with disabilities or chronic illness, living in the community who are isolated, lonely or could use a friend
 
VON - Blood Pressure Clinics - Blood pressure clinics for older adults (55 years and older) with a chronic condition * trained staff and volunteers measure blood pressure, record results and provide information about risk factors for heart disease and stroke as well as materials on how to be more heart-healthy
 
VON Adult Day Program - Spruce Lodge - Adult day program for older adults living in the community (not in Long Term Care) and have a diagnosis of a dementia or other cognitive impairment and would benefit from interacting in a social, recreational and supportive atmosphere * respite for caregiver
 
VON Transportation Services - Non-emergency transportation to medical appointments, grocery shopping, adult day programs, and other appointments for older adults and people with disabilities in North Perth Community by volunteer and staff drivers
