Service Description:
Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurse educators, diabetes educator, chiropodist (foot care specialist), social workers, dietitians, kinesiologist (health professional focused on the movement of the body), and respiratory therapist

Services and programs for registered patients - Primary care, foot care, one-on-one counselling services, chronic disease management, smoking cessation and memory clinic

Services available to both patients and the general public - Health promotion programs on a variety of health topics including heart disease, diabetes, hypertension, cancer, arthritis, and falls prevention * screening programs * physical activity and healthy eating programs * breastfeeding support * virtual prenatal classes.

Telemedicine Program - Working in collaboration with the BAFHT physicians and the Telemedicine Coordinator, the Telemedicine Nurse Practitioner provides primary care services to individuals who require a primary care provider and/or connection with CMHA-GB * program services are available to individuals aged 18 years or older and who have either a serious mental illness or substance use disorder (excluding tobacco as a primary addiction) * referral from CMHA counsellors/workers, ER physician or primary care provider required
